SBA Business Plan Questionnaire Instructions
- If you don’t know the answer to a questions…simply write “I’m not sure”. If the question does not pertain to you, simply write “NA” or “Not applicable”.
- We know you are busy, so try to keep the answers in between 1-3 sentences so we can fully understand the concept. This also allows you to finish in about 15 minutes. You can send us as much information in addition to the questionnaire as you wish.

- What will you call the business? or What is the business name?
- What’s the business phone number and address?
- In what city or state will/does the business operate?
- Are you using the business plan only for the SBA loan application? Will any other parties be reading the business plan? Investors, Employees,etc?
- What product or service will/does your company sell?
- Who are your competitors? Can you provide competitor websites/businesses for reference?
- Why is your business different than your competitors?
- Where does your business find customers to buy the goods and services?
- How much capital (money) are you looking for from a loan or investor?
- How will you spend the capital (money) you raised from the loan or investment?
- About how much do you think the business will make in Years 1, 2, 3, 4, 5 after the loan or investment?
- About how much do you think the business will make in Years 1, 2, 3, 4, 5 after the loan or investment?
- What is the name of the person who will be the CEO? Resume?
- How many owners does the business have? What percentage of ownership does each owner own? What are their names? Resume?
- How many employees will you have? What are their job titles? Resume?
- Describe the business. Tell us anything else you think is important about the business?
